Open Arms is such a great place for my wife and I. The staff are very caring and fun to be with. We also have made some great friends there. Open Arms has truly helped lessen our feelings of loneliness and depression. GOD bless us all.

Aging is taking its toll on us. My husband's visual impairment and my own heart condition have limited our opportunity for socialization. We have started spending more time in bed rather than wake up early and do some physical exercise. We are becoming lazy, so to speak, and we are becoming dependent on our caregivers, thanks to the In-Home Support Services Program. All these have changed since our Primary Care Physician (PCP) suggested that we explore the possibility of participating in an Adult Health Day Care Program.

Mental health experts as well as gerontologists or those who are engaged in the science of aging and longevity say that one of the best ways to prevent depression among the elderly is to expand your circle of friends and to socialize and reach out to others. As one who is definitely advancing in age, I try my best to be active, to reach out to others, and to meet new friends. I would like to remain mentally alert, physically fit, emotionally stable and socially connected. Theseare the reasons why I decided to participate in the Open Arms Adult Health Day Care Center program. Of course, there are other Adult Health Day Care Centers in the County of SanDiego, I encouraged my peers with chronic ailment and other health challenges to look into this invaluable health resource, if they would like to live the remaining days of their lives in a convivial and caring environment.

Horizons Adult Day Health Care Center is a Licensed Community Based Adult Services program (CBAS) that provides services to the elderly and adults with illnesses and or disabilities.


This program is individualized for each participant, based on his or her needs and plan of care. Independence, self-expression and being part of a group helps adults, who may be isolatedby illness, disability or loss, maintain a positive orientation on life.

By focusing on maintenance of physical and mental health, Horizons ADHC enables the participant to maintain as high a level of functioning as possible, thereby allowing them to remain in their homes and communities as opposed to being institutionalized.

We strive to bring a balanced approach to all care provided by listening to and attending to each individual’s unique personal needs.

At Horizons ADHC, complete wellness is our goal—inside and out.

Caring for a loved one with a debilitating disease such as Alzheimer’s Disease, Stroke or Parkinson’s Disease, requiring constant attention and supervision is exhausting! Using CBAS to supplement the care you provide at home allows the family/ caregiver respite time. CBAS also allow their loved ones to remain at home as long as medically, socially and economically feasible.

Blood Pressure can be monitored daily when your loved one is at the center.

Individuals may be eligible for CBAS if they meet one or more of the following criteria:

Individuals who have an Organic, Acquired or Traumatic Brain Injury and/or Chronic Mental Illness.

Individuals with Moderate to Severe forms of Alzheimer’s Disease or other Dementia.

Individuals with Mild Cognitive Impairment including Moderate Alzheimer’s Disease or other Dementia.

Individuals with Intellectual Disabilities and Meet the Criteria for San Diego Regional Center Eligibility.

Rehabilitate and/or maintain participant’s level of care to the maximum extent possible.

Maintain the participant in the community for as long as possible.

Assist participant’s family/caregivers by providing respite care to their loved ones.
