Restore or maintain optimal capacity for self-care to frail elderly persons or adults with disabilities; and Delay or prevent inappropriate or personally undesirable institutionalization.

Glenoaks ADHC is a community based program providing a variety of medical, therapeutic, rehabilitative programs, recreational programs and services for the frail and elderly. Our center offers a full range of structured services including nursing care, social and recreational activities, exercise program and a range of rehabilitation programs that include physical, occupational and speech therapist. Glenoaks ADHC was founded in 2001 to promote the well-being of elder and disabled individuals who live in the area in order to maintain them at, restore them to, their optimal functioning potential and to allow them to remain in the community. Our Center shares with families and others the responsibility of providing substantial care to these individuals by offering respite care from two to five days a week. Our goals are to foster independence of our participants and to enrich the quality of their lives.
